Key Responsibilities Include:

  • Assists the Eskasoni Community Health Teams (Primary Care, Community Health, Maternal Child Health and Mental Health) in developing and implementing a comprehensive diabetes management perinatal program for clients;
  • The Diabetes Nurse Educator is responsible for, and provides, direct clinical services, including screening, education, assessment, intervention, supervision and consultation with clients.
  • The Diabetes Nurse Educator provides individual, family and community education and counselling in diabetes prevention and management that is culturally appropriate within the Eskasoni First Nation.
  • Assists in the recruitment of specialized professional services (internal and external to the community) that will ensure that the identified needs, and service gaps of the client are being met.

Successful Candidates will have:

  • BScN, from an accredited university.
  • Current registration with the College of Registered Nurses of Nova Scotia.
  • Certified Diabetes Educator (CDE) or willingness to obtain within 2 years.
  • Strong knowledge of current diabetes care and treatment clinical guidelines.
  • Ability to work effectively in fast paced growing community health care facility.
  • Ability to wear many hats and function effectively on a multi-disciplinary team.
  • Excellent interpersonal skills; organization ability, and written and verbal communication skills.
